This page provides links to MDOT SHA standard specifications that are required for all construction under an access permit or a district permit.
MDOT SHA Landscape Reporting Form (PDF, 784 KB)

Form is used to report the application of landscape materials and other work on MDOT SHA property. The LRF replaces ‘Pesticide Application Reporting Form’ and ‘Nutrient Management Reporting Form’.
MDOT SHA Modification Request Form (PDF, 170 KB)

This form is used to obtain approval for modifications that do not involve E&S controls in lieu of any specified Modification Request or under Cat. 700 SPs dated 2024.
